Standalone blank template
Use the two-page declaration model when your packaging scope and supporting assessment are ready. Replace the bracketed fields and remove the completion note before issuing.
Complete the evidence before signing
- Identify the manufacturer and packaging type. The manufacturer responsible for the declaration may differ from the EPR producer. Define the packaging covered and its traceable specification.
- Check which requirements apply. Record the relevant use, materials and application dates. Later requirements should not be presented as already met.
- Prepare the technical file. Collect supplier information and assess the evidence under Annex VII. A blank template or supplier statement alone does not demonstrate conformity.
- Complete, sign and control the declaration. Use the Annex VIII structure, confirm the signatory’s authority and required languages, and keep the declaration updated.

Questions about the template
Is this an official EU-issued template?
No. This is EcoComply’s editable resource following the model structure in Annex VIII of Regulation (EU) 2025/40. The regulation is the authoritative source.
Does downloading or completing it make my packaging compliant?
No. The manufacturer must demonstrate conformity with the applicable requirements and prepare the supporting technical documentation before drawing up the declaration.
Can I copy the completed example?
Use it to understand the format only. Its company details, specifications and evidence references are fictional. Replace them with verified information for your packaging.
Is the declaration number an EPR registration number?
No. The declaration identifier is a document reference. Packaging EPR registration is a separate obligation assessed for the relevant market and supply chain.
Does each product SKU need a separate declaration?
The declaration is drawn up for each packaging type. Several SKUs may share a declaration only where the same conformity-relevant packaging type is covered.
Can I use this English version in every EU country?
Check the language or languages required by the Member State where the packaging is placed or made available on the market. An English template does not remove translation requirements.
